If an order is booked to FANUC Robotics America within two years of the original lead registration, you receive $1,000 (normal withholding taxes will apply). Booked means purchase order (PO) received with value greater than $30,000, including at least one new robot.

If an order is booked (purchase order received) through an integrator within two years of the original lead registration and the value of the order is more than $20,000 but less than or equal to $30,000 including one robot, you will receive $500 (normal withholding taxes will apply). If the value of the integrator order is greater than $30,000 including one robot, you will receive $1,000.

In order for you to receive compensation for exclusively providing FANUC Robotics with a product or project lead, the following requirements must be met:

• The individual supplying the lead must supply it exclusively to FANUC Robotics.
• The individual supplying the lead may be required to participate in the sale as required by FANUC Robotics.
• To qualify, an order must be booked (Purchase Order received) within two years of the original lead registration, must have a purchase order of value greater than $30,000 and must include at least one new robot.
• An order booked through an integrator must be booked within two years of the original lead registration and must have a purchase order of value greater than $20,000 including one robot.
• Employees of the referred customer or their family members are not eligible for this program.

In order to qualify for compensation under the new customer referral incentive program, a lead must be a project of which FANUC Robotics is not aware. In addition, FANUC Robotics reserves the right to refuse pursuing leads for any reason, including without limitation the end use, the end user, and/or the source of the referral.

• No money whatsoever will be paid without complete and accurate information on file at FANUC Robotics America, Inc. ("FRA"). Minimum information required of any party receiving a payment: completed and executed Form W-9 or applicable equivalent form.
• FANUC Robotics America, Inc., will file a form 1099 with the US government treating the incentive as a commission.

FANUC Robotics will respond to each individual supplying a lead within thirty (30) business days of receipt indicating whether the lead supplied meets the initial qualifications for compensation under the new customer referral incentive program. Provided that all of the criteria listed in these guidelines has been met, compensation and rebates will be made within forty-five (45) days of FANUC Robotics’ receipt of final payment from the customer according to the following:

If all above criteria is met, FANUC Robotics will pay $500 or $1,000 to the qualifying individual who submits the new customer referral form.

Any employee of FANUC Robotics America, Inc. and their family members are excluded from participating in this program.
